If you are a new patient to our clinic, please arrive at least five minutes before your consultation or fill out a new patient form here and bring the following to your consultation:
- Photo ID
- A valid Medicare card
- Workcover patients will need to inform their employer and bring their employer’s contact details for verification to arrange payment.

Billing Policy
Armstrong Creek Doctors is now a fully bulk billing clinic.
All eligible consultations are bulk billed directly to Medicare. Patients with a valid Medicare card will not incur any out-of-pocket consultation fees. Bulk billing applies to all standard consultations. Please ensure you present your current Medicare card at each visit.
Patients who fit our eligibility criteria will be bulk billed.
Note: Fees may apply for services not covered by Medicare, such as certain procedures, medical reports, or non-Medicare services.
Bulk Billing is available for the following patients:
- Children under 16 years of age
- Patients aged 65 and over
- Pension Card and Healthcare Card holders
Fees may still occur for procedures not eligible for Medicare rebates:
Item 23 – Level B (Standard): $82.85 (Medicare Rebate of $42.85)
Item 36 – Level C (Long): $132.90 (Medicare Rebate of $82.90)
Item 44 – Level D (Prolonged): $182.15 (Medicare rebate of 122.15)
After Hours items:
Item 5020 – Level B (Standard): $125.80 (Medicare rebate of $57.15)
Item 5040 – Level C (Long): $165.70 (Medicare rebate of $98.00)
We occasionally engage the services of locum GPs when our regular doctors are on leave. This may result in variation to our fees.

Introducing in our Practice
Armstrong Creek Doctors is a practice. Most consultations are time-based and will incur private fees for patients who are not eligible to be bulk-bulk under the practice criteria. Fees are payable at consultation by credit card or EFTPOS.
Patients who fit our eligibility criteria will be bulk billed.

Bulk Billing is available for the following patients:
- Children under 16 years of age
- Patients aged 65 and over
- Pension Card and Healthcare Card holders

* Cancellation Policy:
Any changes or cancellations made within two hours of the scheduled appointment time will incur a fee of $40.00 or $50.00 on Public Holidays. This also applies to missed appointments. Subsequent bookings will not be permitted until the fee has been paid.

All patients will be billed privately during our Public Holiday clinics.
This includes concession card holders and children.
Please note: Phone consultations will not be available during these times.

WorkCover & TAC Appointments
Current policy regarding WorkCover and Transport Accident Commission (TAC) consultations:
- Upfront Payment Required: Payment in full is required at reception for all WorkCover or TAC appointments.
- Reimbursement Process: Patients will receive a receipt and tax invoice to lodge a reimbursement claim with their provider.
- Patient Responsibility: The clinic does not provide direct billing to third-party insurers.
WorkCover patients must notify their employer before attending and bring the employer’s contact details.
Afterhours Consultations
DoctorDoctor provides access to bulk billed in home after hours medical care for unexpected illness and may connect patients who do not have a regular GP to daytime General Practice for their ongoing continuity of care. Bookings are from 6pm Monday to Friday, 12pm Saturday, all day Sunday and Public Holidays. Call 13 26 60.
If your situation is an emergency, please call Ambulance – 000.
If you or a loved one needs urgent medical care on a weeknight or weekend, but we are closed, call 13SICK (that’s 13 7425) to request a bulk billed*, after hours doctor home visit.
Our Doctors are on the road from 6pm weeknights, 12 noon Saturday, all day Sundays and public holidays. More GPs trust 13SICK to care for their patients in the after hours. * if eligible for Medicare rebates.
Practice Policies
Correspondence
Medical information in an electronic format is transmitted via encrypted format using secure messaging software. Medical information can be mailed only through secure post (i.e. with a tracking number). Patient diagnostic results and correspondence, whether in electronic or hard copy format is always forwarded to the relevant doctor. The word “Confidential” is recorded on the correspondence. Patient information is only sent via email if it is securely encrypted. We do not accept patient enquiries or requests via email. We also do not send patient information to personal email addresses.
Smoking Policy
This practice has a strict no-smoking policy.
Results
Test results are usually available within 2 – 5 working days unless advised otherwise. Generally, results will not be provided over the telephone. We ask that you make an appointment to discuss all test results. Urgent abnormal test results will be brought to your attention by a doctor or one of our staff members.

We value our patients concerns, so please leave a clear message with the reception staff and your doctor will return your call when possible.
Additional Information
Each staff member is bound by the signed privacy clause contained in the employment agreement. All information received during a consultation between a doctor and the patient is considered personal and private health information. Medical information includes past medical and social history, current health issues and future medical care. Doctors, staff and contractors maintain the privacy of personal information. Medical records are the property of the practice. Only written requests for access to medical records are accepted.
Accessibility
If you require a translator or other assistance with communication, please notify reception when you make an appointment. A phone interpreter may be available but will need to be arranged prior to your appointment. If you require an Auslan sign language interpreter during your appointment, a booking can be made online at http://www.nabs.org.au/ or by phone 1800 246 945.
Computerised Records
Specific systems have been put in place to protect the privacy, security and integrity of patients’ records. All staff are trained in computer security. IT Support oversee maintenance of computers and their security.
Facsimile
Facsimile, printers and other electronic communication devices are located so they are only accessible to the doctors and other authorised staff. All faxes containing confidential information are only faxed to numbers after checking the fax number is correct.
